Enabling Self-Management for Life

Our current predominantly reactive health care system, set up to deal with acute illness and accidental injury, needs to change the way that it engages with patients, the users of the service.

These changes involve keeping people living with long-term health problems as healthy as possible through planning, proven strategies and management to improve the way health services are designed and operated so that they better support self-management. Fully engaged patients, new skills, new care-pathways and job-roles are fundamental to the success.

People with long-term health conditions self-manage all the time, they are the ones who make daily decisions which affect their health, but the way health services support them significantly impacts on the decisions they make.

The Expert Patients Programme CIC can work with you in a number of ways, by helping you to:

  • Really view the services you offer from the eyes of ordinary users of the service
  • Deliver your services in a way which makes self-management the ‘way it’s done here’
  • Optimally integrate tools and enablers designed to increase self-management
  • Train existing and new staff to take on new roles and ways of working which are supportive of self-management
  • Support you to work with patients as ‘role-models’ in new ways, to enable them to support their peers
  • Provide and deliver services for you, designed to support self-management in individuals who currently lack motivation and confidence to self-manage effectively.
